Narrative[]
May 13th, 1935
With war now apparent between the Salieri and Morello crime families, Don Salieri resolves to have Sergio, Don Morello's younger brother and underboss killed. When Sam and Vincenzo fail to kill him in a car chase in the night, Tommy is dispatched to carry out the job. He starts out first with a car bomb recieved by Vincenzo where he is instructed to rig it into Sergio's car, but the attempt fails and is taken out on a young woman instead. Shaken up on inadvertently murdering an innocent person, Tommy calls Vincenzo, who informs him that Sergio is hiding in a diner down the street. He meets with Sam and Paulie armed with Tommy guns and goes in to assassinate Sergio, only to botch it as Sergio used an innocent waitress to cover himself. After a short firefight, Tommy chases Sergio on a motorcycle throughout the city to the docks, with Sam and Paulie following, though both later fell flat. Alone, Tommy guns down the entirety of Sergio's army then encounters the younger Morello brother, who has been revealed to have been shot. Sergio boasts about his luck when Tommy's gun fails to fire, but Tommy comments that he wasn't very lucky at all and executes Sergio by burning him alive when the latter was cornered in flammable barrels, then leaves before the factory explodes.

Completing this chapter grants the When God Stops Smiling achievement, and opens up The Waterfront Post Card mission.

- In contrast to the original game, when the unnamed woman was about to start the rigged car, Tommy is more worried and desperately tried to stop her, and is horrified by the scene, while he made a more darkly humorous remark in the original.
