The Underboss is a position within the leadership structure of Mafia crime families that is second in command to the Boss. The Underboss is often a family member who will take over the family if the boss is sick, killed, or imprisoned. The power of an Underboss greatly varies; some are marginal figures, while others are the most powerful individuals in the family.

Traditionally they run the day-to-day affairs of the family, and receive monetary compensation in various ways. They may be a partner in several rackets and thus get a cut, and in some cases the capos may pass their envelopes through the Underboss, who takes a percentage and passes the remainder to the boss.

In non-traditional crime families an Underboss may be independent of the family, often being the boss of their own crime family or gang, who answer to them. They typically control a number of rackets or territory and are allowed to operate them as they see fit, so long as they pass a percentage of their earnings up to the boss.
